There are four donation options for donating your name to secure your plaque at the Centre for Clinical Haematology.
* Your first donation of £50 will be made when you first fill in the form. When your plaque has been installed for 12 months you will be contacted by the Cure Leukaemia team to make your payment for the second 12 months. This process will repeat 12 months after that to complete your three-year cycle.
If you have chosen to donate via the instalments option and the card you used for the initial payment is due to expire in the next two years we will be in touch to take your future donations manually.
Please note that your donation will appear as WWW.L2PREVOLUTION.COM on your banking statement.
If you make your donation through other means the Cure Leukaemia team will supply a booking code to complete your donation. You are still required to fill in all the fields in the form apart from the payment details for your payment to be processed.
The limit for your name or message is 34 characters to ensure this fits on the plaque.

Once your donation has been made, no refunds are possible unless validated by the team at Cure Leukaemia.

Once your donation has been processed and verified the Cure Leukaemia team will be in touch to confirm your donation and inform you when your plaque will be installed at the Centre. Your plaque will then be in situ for 1 or 3 years from the day of installation depending on the value of donation. When your plaque is in its final year you will be contacted by Cure Leukaemia and given the option to renew your donation to remain installed in the Cure Leukaemia Family Wall.
